Case Managers, Social Workers, GPs, Public Health and Aged Care professionals can now register to refer clients for additional, non-clinical support via local support crews with Connect with Gather My Crew (within Bayside, Yarra Ranges and Greater Shepparton areas).
The online registration process is simple, and, once approved, you’ll be able to refer clients who need support with garden maintenance, occasional meals or some much-needed social connection.
Who are we targeting for garden maintenance?
The elderly and people with limited mobility are an obvious option – but garden maintenance, where neighbours will come by at an agreed time to tackle the lawn or weeds while having a casual chat, is also a great way to access social connection for clients who may be reluctant to accept social support in more ‘traditional’ forms.
Who could do with some meal support?
Everyone could use a night off cooking once in a while – but this support is targeted towards people who may not be quite ready for meals on wheels but could use assistance with a good, nutritious meal once in a while. Pension payments dont always stretch far enough to ensure regular nutritious meals so a weekly meal from a neighbour as part of a support crew may help stretch those payments just a little further – and help connect them to their local neighbourhood.
Who needs extra social connection?
Social isolation is a huge problem within our communities – particularly for our elderly. Local support crews are available for a phone chat, cuppa, walk around the block or catch up over the front fence. It is easy, local and determined by the needs and wants of the individual.
